Setting and Achieving Strategy

In 1989, Hiparcos, a European star-mapping stellite, failed to enter its correct orbit when a rocket motor failed to ignite.
Enquiries revealed that the detonator used to fire the motor had not been modified to work in a vacuum.
The manufacturer had not been told what the detonator was going to be used for.
This example illustrates the importance of having a clear strategy and communicating it to all concerned.
